Among the thyroid hormones produced in the thyroid gland T3 (triiodothyronine) is regarded as the most active, and is produced (up to 80%) by the breakdown of the other major thyroid hormone T4 (tetraiodothyronine) in the body’s tissue.

The majority of both thyroid hormones in the bloodstream are in the protein-bound form, and a minority are unbound or Free. Variations in total thyroid hormone in bloodstream may result from either thyroid hormone production or changes in the bound concentrations.

Thyroid disorders usually appear when there is a net change of free unbound fractions over a period of time, therefore the best measure of thyroid status is the concentration of both free hormones. Serum level of free T4 (FT4) correlates very well with secretion and metabolism rate of T4.

Free T4 levels have been recommended as the most reliable and meaningful indicator of thyroid diseases, especially in conflicting or borderline instances.
