Among the thyroid hormones produced in the thyroid gland T3 (triiodothyronine) is regarded as the most active, and is produced (up to 80%) by the breakdown of the other major thyroid hormoneT4 (tetraiodothyronine) in the body’s tissue.

The majority of T3 (99.7%) in the bloodstream is the protein-bound form, and a minority of T3 (0.3%) is unbound.

Variations in total thyroid hormone in blood may result from either changes in the bound concentrations, or thyroid hormone production. Thyroid disorders usually appear when there is a net change of free unbound fractions over a period of time, therefore the best measure of thyroid status is the concentration of both free hormones.

Determination of the free T3 concentration allows also the follow-up of patients under liothyronine therapy.
